I am currently receiving ongoing prenatal care. I have undergone a medical diagnostic ultrasound prescribed by my OB provider for this pregnancy. I understand that my OB provider is responsible for confirming my due date, screening for fetal abnormalities, and addressing any medical concerns related to my pregnancy

Baby's First Smile is not a medical provider and does not perform any diagnostic services. I understand that this ultrasound session may last no more than 25 minutes and will focus on the pelvic area. I acknowledge that this is a limited, non-medical ultrasound and does not replace any diagnostic ultrasound ordered by my OB provider. Baby's First Smile is not a healthcare provider. We recognize the importance of proper prenatal medical care for both the expectant mother and the fetus. To provide a meaningful keepsake ultrasound experience, we require that clients: Certify they are under the care of a physician or healthcare provider and are not using this ultrasound as a substitute for standard prenatal care.

Notify their physician or healthcare provider about the keepsake ultrasound received from Baby's First Smile. We request that clients present a signed acknowledgment of this notice from their physician or healthcare provider prior to the ultrasound session. If this is not possible, clients assume responsibility for notifying their provider afterward

By receiving services from Baby's First Smile, I acknowledge and agree to the following: This ultrasound is an elective, non-medical procedure that I have voluntarily requested. It is not intended to replace any diagnostic ultrasound or medical test recommended by my healthcare provider. Because this is a non-medical service, it is generally not covered by insurance. Payment is required in advanc

The ultrasound technician is not a doctor, nurse, or healthcare provider and cannot interpret or diagnose medical conditions. The purpose of this ultrasound is to provide enhanced images for viewing fetal movement. No medical evaluation will be performed. I am responsible for contacting my healthcare provider with any questions or concerns.

The quality of the ultrasound and any media (DVD, digital files, etc.) depends on factors such as body tissue, fetal development, and position. Baby's First Smile does not guarantee the quality of the media or the visibility of fetal features. Any video recordings not provided by Baby's First Smile may not be published, presented, or distributed.

All images and video clips from my session may be used for promotional purposes by Baby's First Smile. I agree to receive emails and SMS messages from Baby's First Smile, including surveys, promotional offers, and marketing. I consent to receive my session’s images and videos via email and smartphone

I understand that factors beyond Baby's First Smile’s control may affect the accuracy of gender determination, and no guarantees are provided. I also acknowledge the potential, though currently unknown, risks of ultrasound exposure, even though it is believed to be safe

Release of Liability In consideration of the services provided, I release Baby's First Smile, its agents, affiliates, members, managers, and employees from any and all claims or liabilities arising from this keepsake ultrasound, including but not limited to: Incorrect gender determination Undetected fetal characteristics or anomalies Any presently unknown risks associated with ultrasound
